Aching knees, a stiff back after a long day at the desk, sore shoulders after the gym: joint and muscle pain is one of the most common reasons people look for help. RK Xero is a herbal capsule that many Malaysians ask us about. This guide explains what is in it, how to take it, who should check with a doctor first, and when pain needs more than a capsule.

In short: RK Xero is a traditional medicine registered with the Ministry of Health (MAL23076107TC). It contains nine plant extracts, including boswellia, turmeric and ginger, and is traditionally used to alleviate joint and muscle pain. Adults usually take 1 capsule twice a day after meals. Ask a doctor first if you take blood thinners, and see a doctor for pain that is severe, new or not settling.

What is RK Xero?

RK Xero is a capsule made by RK Global Pharma. Each box holds 30 vegetable capsules (3 blister strips of 10). It is registered as a traditional product, which means its registered use, "traditionally used to alleviate joint and muscle pain", is based on how its ingredients have long been used. Traditional products are not tested in the same large trials as prescription medicines, so it is best thought of as a gentle option for everyday aches, not a treatment for a diagnosed disease.

At CarePlus Clinic it is sold over the counter: see RK Xero, RM129 for 30 capsules.

What is inside RK Xero

  • Boswellia serrata (Indian frankincense): a resin used in Ayurvedic medicine for joints. Its boswellic acids are the most studied part of the formula.
  • Turmeric (Curcuma longa): contains curcumin, studied for joint comfort.
  • Black pepper (Piper nigrum): its piperine helps the body absorb curcumin.
  • Pineapple stem (Ananas comosus): the source of bromelain, an enzyme traditionally used for swelling.
  • Ginger (Zingiber officinale): a warming spice used for aches and stomach comfort.
  • Onion (Allium cepa): contains quercetin, a plant antioxidant.
  • Cissus quadrangularis: a vine used in traditional medicine for bones and joints.
  • Chilli (Capsicum annuum): contains capsaicin, the same compound used in warming pain creams.
  • Clove (Syzygium aromaticum): contains eugenol, traditionally used for pain.

What the research says

RK Xero itself has not been tested in published clinical trials. Some of its ingredients have:

  • Boswellia: a 2020 review of seven trials in people with knee osteoarthritis found that boswellia extracts eased pain and stiffness and improved movement compared with placebo, with few side effects. The trials were small.
  • Turmeric and curcumin: studies suggest a modest effect on joint pain, but the evidence is limited, according to the US National Center for Complementary and Integrative Health.
  • Bromelain, ginger and capsaicin: each has some early research for pain or swelling; most studies are small.

So the honest summary is: a sensible traditional option for everyday aches, with some supporting research on its main ingredients. It does not replace a diagnosis, physiotherapy or medicines your doctor prescribes.

How to take RK Xero

  • Adults: 1 capsule twice a day after meals. For stronger pain the pack allows up to 2 capsules twice a day.
  • Swallow with water. Taking it after food helps avoid stomach upset.
  • When the pain settles, take a lower dose or stop.
  • If you are not better after 2 weeks, see a doctor.

Who should ask a doctor first

  • People taking blood thinners such as warfarin, aspirin, clopidogrel, apixaban or rivaroxaban: turmeric, ginger, bromelain and clove may increase the risk of bleeding.
  • Anyone with gallstones, a stomach ulcer, bad reflux or a bleeding disorder.
  • People having surgery or a tooth extraction: stop 2 weeks before.
  • Women who are pregnant or breastfeeding, and children.
  • Anyone allergic to pineapple, turmeric, ginger, chilli, clove, onion or pepper.

Common causes of joint and muscle pain

  • Muscle strain from lifting, sport or a sudden movement. Usually better within days to weeks.
  • Desk and posture pain in the neck, shoulders and lower back.
  • Wear-and-tear knee or hip pain (osteoarthritis), common after 45, worse with weight and stairs.
  • Overuse, such as tennis elbow or a sore wrist from typing.
  • Gout, a sudden, very painful, red and swollen joint (often the big toe). This needs a doctor and a uric acid test.

Simple steps that help alongside

  • Keep moving gently. Short walks and stretching help more than complete rest.
  • Use a warm pack for stiff muscles, or a cold pack for a new injury in the first 48 hours.
  • Strengthen the muscles around the joint; a physiotherapist can show you how.
  • Losing even a few kilograms takes pressure off the knees.
  • Check your desk set-up: screen at eye level, feet flat, a break every 30 to 45 minutes.

When to see a doctor straight away

  • A joint that is red, hot and very swollen, or pain with fever.
  • Pain after a fall or injury, or if you cannot put weight on the leg.
  • Back pain with numbness or weakness in the legs, or problems passing urine.
  • Pain that wakes you at night, or comes with weight loss you cannot explain.
  • Pain that is not better after 2 weeks.
