Mounjaro is one of the most searched medicines in Malaysia. It is a once-weekly injection that can lead to significant weight loss and better blood sugar control, but it is a prescription medicine that is only safe when a doctor has checked it suits you. This guide explains what Mounjaro is, who it is for, how the doses work, what results to expect, the side effects, and how to make sure you get a genuine pen.

In short: Mounjaro (tirzepatide) by Eli Lilly is a weekly KwikPen injection for adults with type 2 diabetes, and for weight management in adults with a BMI of 30 or more (or 27 or more with a weight-related condition). You start at 2.5 mg and step up every 4 weeks. Nausea and other stomach upsets are common early on. Get it only through a doctor, and check the pen is a registered product with a FarmaTag hologram.

What is Mounjaro?

Mounjaro is the brand name of tirzepatide, made by Eli Lilly. It comes as a pre-filled KwikPen that holds four weekly doses. Tirzepatide copies two natural gut hormones, GIP and GLP-1. Together they help the pancreas release insulin when blood sugar is high, slow down how fast the stomach empties, and act on the brain to reduce hunger. Most people feel full sooner and think about food less.

It is different from semaglutide (Ozempic and Wegovy), which acts on GLP-1 only. In a head-to-head trial in people with obesity, tirzepatide led to more weight loss than semaglutide.

Who is Mounjaro for?

  • Type 2 diabetes: adults whose blood sugar is not controlled well enough with diet, exercise and other medicines.
  • Weight management: adults with a BMI of 30 or more, or a BMI of 27 or more with a weight-related health problem such as high blood pressure, prediabetes or diabetes, high cholesterol, fatty liver or sleep apnoea.

Asian bodies tend to develop diabetes and heart risk at lower weights, so Malaysian guidelines use lower BMI cut-offs for overweight (23) and obesity (27.5). Your doctor will look at your whole health picture, not just one number.

Who should not use it

  • anyone with a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2 (MEN2);
  • women who are pregnant, trying to conceive or breastfeeding (stop at least 1 month before trying for a baby);
  • people with type 1 diabetes, or a history of pancreatitis, need specialist advice;
  • anyone with severe stomach problems such as gastroparesis.

How the doses work

Mounjaro is started low and increased slowly so your stomach can adjust.

WeeksWeekly doseWhat happens
1 to 42.5 mgStarting dose to get used to the medicine; not meant for full effect
5 to 85 mgFirst treatment dose; many people stay here if it works well
Then, if needed7.5 mg, 10 mg, 12.5 mg, 15 mgIncreased in 2.5 mg steps, at least 4 weeks apart

Inject once a week on the same day, at any time, with or without food, under the skin of the tummy, thigh or upper arm. Change the spot each week. If you miss a dose and 4 days or less have passed, inject as soon as you remember; if more than 4 days have passed, skip it and take the next dose on your usual day.

A higher dose is not automatically better. Many people do well on 5 mg or 7.5 mg with fewer side effects. Your doctor will only increase the dose if you are tolerating it and need more effect.

What results to expect

  • In the SURMOUNT-1 trial of adults with obesity without diabetes, people on the highest dose lost on average about 20% of their body weight over 72 weeks, compared with about 3% on placebo, when combined with diet and exercise.
  • People with type 2 diabetes usually lose a little less weight, but their HbA1c (3-month blood sugar) often falls by around 2 percentage points.
  • Weight loss is gradual. Most people see the scale move in the first month, with the biggest changes between months 3 and 12.

Stopping matters: in trials, people who stopped tirzepatide regained much of the weight within a year. Think of it as a long-term treatment, used alongside eating and activity changes that you can keep up.

Side effects

The most common side effects are in the gut, and they are usually worst in the first few days after starting or increasing the dose:

  • nausea, burping, bloating and reduced appetite;
  • diarrhoea or constipation;
  • vomiting and indigestion;
  • tiredness and mild injection-site reactions.

See a doctor straight away for severe tummy pain that spreads to the back (possible pancreatitis), pain in the upper right tummy or yellow eyes (gallbladder problems), signs of dehydration, or a severe allergic reaction. Read our full guide to Mounjaro side effects and how to manage them.

Important for women: Mounjaro can make the oral contraceptive pill less reliable. Use condoms as well for 4 weeks after starting and for 4 weeks after each dose increase, or talk to your doctor about other contraception.

What affects the cost of Mounjaro in Malaysia

The price of a pen depends on the strength, and one pen lasts four weeks. When you compare prices, check what is included: a doctor's assessment, follow-up as you step up the dose, cold-chain delivery and pen needles. The cheapest offer is not a saving if the pen is fake or has been stored warm. For the current price of each strength at CarePlus Clinic, see our Mounjaro KwikPen page or WhatsApp us on +60 11-2615 1500.

How to avoid fake or unsafe weight-loss pens

The Ministry of Health has warned that weight-loss injections are being sold online without prescription, examination or monitoring, including pens claiming to contain retatrutide, a drug that is not registered in Malaysia. To stay safe:

  • Get it through a doctor who checks your health, explains how to inject and follows you up.
  • Check the pack for the FarmaTag hologram and verify it with the FarmaChecker feature in the MyUBAT app.
  • Avoid marketplace and social media sellers, unlabelled vials, "compounded tirzepatide" and anything sold without a prescription.
  • Keep the cold chain: unused pens must stay in the fridge at 2 to 8°C and must never be frozen.
